Asking for a friend new to the island, How and Where do you go to get a Health Card, and what documents(driver's licencse, SS, etc?) do you need to bring with you. Thanks.

I think I remember....going to the clinic in Sunny Isle where you fill out a form and get the vial for the "sample".
Once you turn in the sample they run the tests and you wait about a day or two. They then give you forms saying that you're worm free and you take that to the main Health Dept. Bypass the waiting room and take the hallway on the left and one of the first rooms on the left is for food service workers (I think).
The lady imputs your info, checks your ID (my Colorado license worked), and snaps your photo. Then she issues a recepit that you take to the cashier near the waiting room.
I think it ran about $50 for the whole shebang and took me 10 minutes at each location. My boss told me that I shattered all VI time records.
I could be missing a step, but that's what I remember!
